Adherence to Tuberculosis Treatment in Paraguay: a Challenge with a Human Face. 2022 Analysis

Abstract
Introduction: Tuberculosis (TB) remains one of the leading causes of death worldwide and a public health problem in Paraguay, where non-adherence to treatment affects 16% of patients. This study aims to identify the factors limiting therapeutic adherence in patients with drug-sensitive TB in Paraguay.
Methodology: A retrospective, cross-sectional, analytical study was conducted using a structured questionnaire applied to 157 TB-diagnosed patients across three health regions between January and May 2022. Adherence was assessed using the Morisky-Green test and analyzed through univariate logistic regression.
Results: 37% of patients were non-adherent to treatment, with significant barriers identified, including inadequate clinic hours (OR = 6.66, p < 0.05), long waiting times (OR = 4.4, p = 0.009), and difficulties obtaining work permits (OR = 5.59, p = 0.04). Sociodemographic and epidemiological characteristics showed that 56% of patients were unemployed during treatment, 35% lived in precarious conditions and 87% were not under directly observed treatment.
Conclusions: The complexity of TB treatment adherence in Paraguay is highlighted, emphasizing the need for comprehensive strategies involving multisectoral actors to address healthcare access barriers and consider socioeconomic factors. The information gathered is crucial for developing health policies that improve patient quality of life and contribute to effective TB control in the country.
