Whole Lung Lavage: a Comprehensive Review of Techniques, Efficacy and Outcomes

Abstract

Whole lung lavage (WLL) is a well-established therapeutic procedure for the management of pulmonary alveolar proteinosis. Despite its long-standing use, questions remain regarding its indications, techniques and outcomes. This comprehensive review aims to provide a detailed analysis of the current state of knowledge on WLL. 


We explore the history and delve into the intricacies of patient selection, pre-procedural assessment and perioperative management. We examine the procedural details in-depth, describe a technique regarding the infusion and retrieval of saline solution, and provide technical recommendations for the entire procedure. Moreover, we will discuss other novel WLL techniques and their respective advantages, limitations, and complications. 


Finally, we will address therapeutic efficacy, highlighting the need for standardized outcome measures and long-term follow-up data. We hope that the proposed technique and technical advice discussed brings us one step forward regarding the standardization of the procedure by exhorting international expert panel discussions and that the ideas mentioned stimulate further investigation into WLL utility and enhancement.
