Impacto del sobrepeso en la evolución clínica de niños hospitalizados por bronquiolitis

Introducción: La bronquiolitis es una enfermedad altamente prevalente y la principal causa de hospitalización en niños menores de 12 meses. Los casos de sobrepeso y obesidad están aumentando a nivel mundial en la población pediátrica. Este estudio tuvo como objetivo determinar el impacto del sobrepeso en la evolución clínica y las complicaciones de los niños hospitalizados con bronquiolitis en un centro de tercer nivel.
Métodos: Estudio observacional, retrospectivo durante 12 meses (enero-diciembre 2019). Los datos se obtuvieron de historias clínicas electrónicas según criterios de inclusión y exclusión predefinidos. El análisis se llevó a cabo con STATA versión 14 y R 4.2.
Resultados: Se analizaron datos de 200 pacientes, con una edad mediana de 5 meses (rango 0-23, DE 5,45); 47 pacientes (23,5%) tenían sobrepeso. Los síntomas más frecuentes fueron dificultad respiratoria (94,5%) y taquipnea (89%). El virus sincitial respiratorio (VSR) fue el patógeno más común (84,1%). Los pacientes con sobrepeso tuvieron una estancia hospitalaria más prolongada (+1 día) y mayores requerimientos de oxígeno (+1 día). También presentaron mayores probabilidades de insuficiencia ventilatoria al ingreso (OR 3,03; IC 1,13-7,85; p=0,009), uso de cánula nasal de alto flujo (OR 2,48; IC 1,18-5,15; p=0,007) y uso de broncodilatadores (OR 2,13; IC 1,02-4,41; p=0,02). El análisis multivariado confirmó estas asociaciones.
Conclusiones: Este es el primer estudio en América Latina que explora la asociación entre sobrepeso y gravedad de la bronquiolitis en niños hospitalizados. Nuestros hallazgos sugieren que el sobrepeso puede actuar como un factor de riesgo, contribuyendo a una evolución clínica más severa.
