Expert Consensus: Clinical Recommendations for the Use of Glucocorticoids in Patients Diagnosed with Asthma

Abstract
Introduction: The use of glucocorticoids is essential in the management of asthma; however, their overuse entails significant risks.
Objective: Develop a multidisciplinary consensus to optimize their use in clinical practice in Colombia and Latin America.
Methodology: The consensus is based on an expanded review of clinical practice guidelines, international consensus statements, and literature reviews. A total of 102 publications were analyzed, of which 24 were included for evidence extraction, recommendations, assessment of evidence quality, and strength of recommendations. These elements informed the drafting of recommendations that were subsequently submitted to voting during the consensus conference.
Fourteen experts representing scientific societies, academia, professional associations, and a patient representative participated in validating the recommendations following a consensus conference methodology.
Results: Resulting clinical recommendations addressed criteria for glucocorticoid use, management of asthma exacerbations, follow-up, and implementation strategies. Emphasis was placed on calculating cumulative doses, identifying alert thresholds, monitoring patients receiving glucocorticoids, and referral to specialists.
This consensus provides recommendations for the rational use of glucocorticoids in patients with asthma, as well as the need for strict monitoring of glucocorticoid use and associated effects, with the goal of achieving asthma control through appropriate therapeutic regimens. It promotes continuous education processes and alignment with international guidelines. The consensus offers practical tools for clinical decision-making and risk management.
