@article{González_Fielli_Sanchez Hinestroza_Santos_Magdaleno_Irurtia_Rampulla_Capece_Posse_Sanguineri_2022, title={Diagnóstico de neumonía por Pneumocystis jirovecii por PCR}, volume={14}, url={https://respirar.alatorax.org/index.php/respirar/article/view/130}, DOI={10.55720/respirar.14.4.3}, abstractNote={<p><strong>Introducción: </strong><span style="font-weight: 400;">la neumonía por </span><em><span style="font-weight: 400;">Pneumocystis </span></em><span style="font-weight: 400;">es una infección oportunista causada por </span><em><span style="font-weight: 400;">Pneumocystis jirovecii (Pj)</span></em><span style="font-weight: 400;"> en pacientes inmunocomprometidos, especialmente en pacientes con VIH.</span><strong> Objetivo:</strong><span style="font-weight: 400;"> describir las características clínicas, radiológicas y la evolución de pacientes inmunocomprometidos con diagnóstico de neumonía por </span><em><span style="font-weight: 400;">Pj </span></em><span style="font-weight: 400;">mediante PCR en lavado broncoalveolar (LBA). </span><strong>Métodos: </strong><span style="font-weight: 400;">se realizó un estudio transversal, descriptivo, en el que se incluyeron pacientes con inmunocompromiso, con cuadro clínico y radiológico compatible con neumonía por </span><em><span style="font-weight: 400;">Pj</span></em><span style="font-weight: 400;">, diagnosticado sólo mediante PCR en muestra de LBA</span><em><span style="font-weight: 400;">. </span></em><strong>Resultados: </strong><span style="font-weight: 400;">fueron atendidos 141 pacientes entre marzo de 2017 y diciembre de 2019 con sospecha clínica y radiológica de neumonía por </span><em><span style="font-weight: 400;">Pj</span></em><span style="font-weight: 400;"> a los que se les realizó LBA. En el análisis se incluyeron 22 pacientes en los cuales se confirmó el diagnóstico únicamente mediante PCR en muestra de LBA. Los signos y síntomas más frecuentes fueron tos (100%) y disnea (77%), y el patrón radiológico predominante fue el infiltrado intersticial y vidrio esmerilado bilateral. En 4 pacientes (18.8%) se observó coinfección pulmonar, con aislamiento de </span><em><span style="font-weight: 400;">Mycobacterium tuberculosis</span></em><span style="font-weight: 400;"> y </span><em><span style="font-weight: 400;">Streptococcus pneumoniae</span></em><span style="font-weight: 400;">. La duración de la estancia hospitalaria fue de 13 días (DE±11). Requirieron ventilación mecánica 6 pacientes (27%). La mortalidad intrahospitalaria fue de 23%. </span><strong>Conclusiones:</strong><span style="font-weight: 400;"> la utilización de PCR en muestras de LBA podría constituir un método útil para el diagnóstico de neumonía por </span><em><span style="font-weight: 400;">Pj</span></em><span style="font-weight: 400;">.
